The Transitions of Care Special Interest Group (SIG) is a professional network of pharmacists and pharmacist extenders working to overcome transitional challenges and enhance collaboration among pharmacists in all practice settings as they provide care to patients transitioning to and from various settings. The Transitions of Care SIG has developed the following resources for those working in the transitions of care field.
